Cash Rebate Request
To receive your cash rebate, follow these simple steps:

Complete the Rebate Reimbursement Request Form.

Submit proof of purchase by attaching receipt to the form.

Submit proof of inspection by your local municipality or county inspection office (required on natural gas water heater, furnace or hydro-heat system.)
Consumer Notice: Under Florida Law e-mail addresses are public records. If you do not want your e-mail address related in response to a public records request do not send electronic mail to this entity. Instead, contact this office by telephone or in writing.
